Informed Consent Form

You are invited to participate in a research study which examines the professional opinions of mental health practitioners about influences on the development of eating disorders. The purpose of the study is to explore mental health practitioners’ views about the various influences on the development of eating disorders in different cultures.
This study is being conducted by Dr. Bengu Erguner-Tekinalp and Dr. Catherine Gillespie.

If you decide to participate, your involvement will only extend to the completion of this survey. We estimate that this will take 15 minutes of your time. There is a minimal foreseeable risk of possible emotional discomfort when filling out the survey or when reflecting on it later. There is no compensation associated with participating to the study. But there may be benefits to the profession, if we are able to further determine the influences on the development of eating disorders in different cultures.

Any information obtained in connection with this study will remain anonymous. The server for the website is located at Drake University. Information collected through your participation will be used for research purposes only and may be published in a professional journal, and/or presented at a professional meeting. The survey is anonymous and we will not trace its source. You may discontinue participation at any time by aborting the survey, without penalty. Once your completed survey is submitted, because it is anonymous, we cannot remove your survey from the others we have received.

Participation is entirely voluntary and will not affect you professionally or personally in any way. Your decision whether or not to participate will not jeopardize your future relations with Drake University or School of Education.
